 1. MULTI-STATE RESALE CERTIFICATE: We require a valid Sales & Use Tax Certificate to comply with tax laws. If 
ComplyRight drop ships products to multiple states on your behalf, we need applicable certificates for each state.
 2. MINIMUMS: $25.00 minimum-net per shipment and per location, plus shipping (excluding rush charges).
 3. RUSH SERVICE: All rush orders received by 7:00 A.M. (EST)/ 10:00 A.M. (PST) will be shipped same day. In addition to 
shipping cost, a $35.00 Premium Service Rush Charge will be billed for stock forms per shipping address.
 4. RETURNS: Unauthorized returns are not accepted. Authorized returns incur a 25% restocking fee (excluding shipping) 
and must be made within 30 days of the invoice date through Dec. 31, 2026. Returns require prior approval and a 
return goods (RG) number. No Forms or Envelope returns accepted after Dec. 31, 2026.
 5. UNDELIVERABLE RETURNS: Freight charges apply to undeliverable shipments or those refused by the consignee. 
Charges equal original freight costs.
 6. ADDRESS CORRECTIONS: Incomplete or inaccurate addresses incur a $21.00 per carton fee for address corrections. 
Additional fees apply for rerouted shipments.
 7. RESHIPMENTS: Reshipments will ship same method as original order. Our liability is limited to the replacement of 
forms only. We will not issue credits on duplicated orders. All claims must be made within 30 days. 
NO credit will be issued on reorders due to freight delays (i.e., weather, acts of God, etc.).
 8. CANCELLATIONS: No cancellations accepted after shipment. A $10.00 fee applies for cancellations after 24 hours if 
the order hasn't shipped. 
 9. TERMS: Our terms are net 30 days from date of invoice. 
10. NEW ACCOUNTS: All orders will be shipped Cash In Advance (Check, M/C, Discover, VISA, American Express 
accepted) until a line of credit is established. Credit checks begin after receipt of application.
11. SHIPPING: All shipments F.O.B. from our nearest warehouse with stock available. WE ARE NOT RESPONSIBLE FOR 
SHIPPING DELAYS ONCE THE FREIGHT HAS LEFT OUR WAREHOUSE. Allow 1–2 business days on stock orders; 
7–10 working days on imprinted form orders and additional time for larger quantities.
12. All Orders received on Saturday will be processed as though ­received on Monday.
13. FORMATS: It’s important that your customer indicates their required format. Samples for test runs are available at no 
charge. For samples, order early to allow time for transit as these are shipping from MN warehouse only!
